Shift: 8:00 AM - 4:30 PM

Experience:

24 months of role experience is required.

12 months of Home Health experience is required.

12 months of travel experience is required.

3 months of experience with Epic is required.

Requirements:

Candidates must have a Massachusetts license in hand at the time of application.

This role may require floating to additional units and locations

Candidates must live at least 50 miles away from this facility in order to be considered a traveler when applying for this role.

COVID vaccination required for onboarding. Declinations not accepted.

Flu vaccination required for onboarding. Declinations not accepted.

Additional Details:

Seasonal Flu Vaccine is required if working during flu season (September 15-April 30th).

Must be fully vaccinated (exemptions are reviewed on a very limited basis).

Required: 1 year Case Management Experience. Ability to travel to cases as assigned.

Also required to perform admission processes and case management responsibilities in accordance with MGB Home Cares policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
