ThisNeuroStroke RN will assist in in the oversight of stroke patients and their continuum of care as they move through the acute care system. This RN must possess impeccable communication skills as they navigatepatient'splan of care with all members of the interdisciplinary team. Responsibilities for thisNeuroStroke RN include acting as a patient advocate, assisting patients in reaching optimal health in a positive environment, directing, monitoring, and evaluating nursing care in accordance with established policies and procedures, and assisting with ongoing patient assessment and admission/discharge processes to ensure high quality, patient-centered care.
