    Based in Louisville, Kentucky, Trilogy Health Services is looking for a Clinical Liaison to join its Home Office team. This role is a flexible, hybrid position and reports to the Division of Business Development Director.
    WHAT WE'RE LOOKING FOR:
    A Clinical Liaison helps to develop, coordinate, direct and administer communications between patients, family, and care professionals. They ensure projects are designed and maintained to Trilogy's standards. This role will help maximize the number of admissions to the health center from assigned hospitals. Other key responsibilities will include:
    • Establish patients' eligibility for care, communicate closely with families, and interact in a positive, professional, respectful, and customer-focused way with a wide array of staff members from case managers to physicians.
    • Conduct pre-admission assessments to gather clinical documentation.
    • Respond immediately to patient referrals from hospital case managers, physicians, and managed care organizations.
    • Facilitate meaningful exchanges with hospital-based case managers on risk factors and care plans.
    • Educate hospital staff about Trilogy Health Services' clinical capabilities and match patients to the best-suited facility and services based on their unique needs.
    • Educate patients and family members about services offered and address questions and concerns prior to admission, as well as explain the spectrum of available care options.
    • Maintain a strong working knowledge and adhere to applicable federal and state regulations including, but not limited to, laws supporting patient confidentiality, release of information, and HIPPA.
    • Review clinical documentation to ensure it supports the insurance requirements for approval.
    • Regularly participate in hospital and ACO meetings to foster deep partnerships with key contacts.
    • Maintain accurate, timely data entry on business activity.

    Qualifications
    • Associate's Degree or equivalent experience - a background in Discharge Planning, Nursing, Case Management, and/or Utilization Management preferred.
    • RN/LPN license preferred
    • 2+ years of relevant experience in a hospital, skilled nursing care facility or other medical facility.
    • Familiarity with area hospitals, physicians, discharge planners, and other referral sources preferred.
    • Experience in Senior Living or Long-term Care preferred.
    • Strong interpersonal and relationship-building skills.
    • Excellent verbal and written communication skills + organization and time management skills
    • Knowledge of electronic health records, medical terminology, and coding
    • A valid driver's license and the means to drive oneself to various community locations and activities.

    Physical Requirements

    Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs. Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus.
