    Position Summary: The NHTD Coordinator will assist the applicants and participants in obtaining and coordinating the services that are necessary to remain in or return to their home. The NHTD Coordinator will serve as the primary contact for the participant in the Waiver Program and will provide ongoing assessment and reevaluation of the participants health and welfare, and a determination of what services are needed to maintain this individual within their home.

    Essential Duties:

    • Development of Participants Application Packet. This shall include, but not be limited to, completion of Individualized Service Plans and Plans of Protective Oversight (PPO); meeting with participants to gain an in-depth understanding of their goals; and meeting with significant individuals, as well as agencies, providing services to establish a comprehensive understanding of the participants need for services.
    • Act as the participants advocate, provide unbiased assistance to the participant with the selection of providers, and where appropriate, assist the participant in locating a place to live in the community.
    • Provide each participant a Waiver Contact List, as well as provide a review of this Contact List to assure it is correct, up-to-date and redistributed on an as-needed basis.
    • Provide assistance to waiver participants in terms of implementing the plan, as well as revieing its effectiveness.
    • Coordination and scheduling of waiver and non-waiver services.
    • Maintain consistent and open communication with the RRDS and waiver participants.
    • Assure timely implementation of the approved service plan.
    • During the first 6 months, conduct face-to-face meetings with the waiver participant monthly at a minimum to monitor health and welfare needs as the participants adjust to the waiver program.
    • After 6 months visits with participants must occur every six months at a minimum.
    • Maintain case records consistent with all contacts and necessary documentation.
    • Maintain a tracking system that monitors all areas of compliance.
    • Practice proper safety procedures and always take precautions to ensure their safety and the safety of others.
    • Reports all safety hazards or concerns to their supervisor in a timely fashion.
    • Always maintains a high degree of confidentiality due to access to sensitive information.
    • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
    • Follows all Medicare, Medicaid, and HIPAA regulations and requirements
    • Abides by all regulations, policies, procedures and standards
    • Performs other duties as assigned

    Position Requirements & Competencies:

    • A masters degree with a minimum of one years experience providing service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors.
    • A bachelor's degree with a minimum of three years experience providing service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors.
    • An associate degree with a minimum of five years experience providing service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors.
    • Excellent oral and written communication skills
    • Ability to multi-task under tight deadlines
    • Strong communication skills and interpersonal skills
    • Valid drivers license and proof of insurance is required

    Working conditions: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to sit, talk, hear; use hands to finger, and handle controls. The employee frequently is required to reach with hands and arms. The employee is occasionally required to stand, walk, and stoop. Specific vision abilities required by this job include close vision and the ability to adjust focus.

    Environment: Moderate noise level similar to a typical office environment with computers, printers, and work activity.
