Shift: 8:00 AM - 4:30 PM

Experience:

36 months of role experience is required.

24 months of Case Management experience is required.

Travel experience is required.

Experience with Epic is required.

Requirements:

Candidates must have a Arizona license or compact license in hand at the time of application.

This role may require floating to additional units and locations

Candidates must live at least 75 miles away from this facility in order to be eligible to apply for this role.

COVID vaccination required for onboarding. Declinations not accepted.

Flu vaccination required for onboarding. Religious and medical declinations accepted.

Additional Details:

Required to work wknds (3 wknd shifts/contract), rotating shifts like evenings, holidays.

CM nurses ensure patients receive quality care that is cost effective; Works directly w/ patients & family to conduct assessments & analyzes data to determine patients needs; Collects data while working closely w/ the patients healthcare team.

Caseload ~20-30 patients/day

Travelers cannot request RTO for both Memorial (5/29/23) and Independence Day (07/04/23). Please only request 1.

If more than 6 days of RTO is needed, add that time to end of contract. Refrain from Holiday RTO

Maximum days off: 6 days
