    Position Summary:
    We have an exciting opportunity to join our team as a Hepatology Nurse Coordinator- Transplant Institute.

    In this role, the successful candidate The Hepatology Nurse Coordinator is an important member of the multidisciplinary Hepatology and Liver Transplant team. The RN will oversee care for hepatology patients in the outpatient setting. Specifically, the RN is responsible for: collecting clinical information about patients, summarizing medical data, ensuring diagnostic testing and treatment procedures for patients were completed; triaging phone calls from patients; providing education to patients and families about liver disease symptoms, management, and diet; interfacing with multiple providers to ensure optimal care. The Hepatology Nurse Coordinator will assist in office operations to ensure efficiency and increased access to hepatology patients referred to the health system. This RN will be responsible for revamping and revising current referral system.

    Job Responsibilities:

    Return triages: email, phone calls (urgent), In-basket messages

    Revamp and revise current referral system.

    Provides excellent patient services to ensure satisfaction.

    Liaisons with the inpatient team and liver transplant team to ensure continuous care for hepatology patients.

    Communicates with the multidisciplinary team to optimize patient care. Interfaces with physicians, social workers, nurse practitioners and others.

    Maintains contact with assigned caseload between visits, based on their particular needs. Initiates appropriate responses to issues and problems during these contacts.

    Ensure laboratory, radiologic, and diagnostic studies scheduled (with FGP medical secretary) and completed.

    Educate patients on diet, liver disease symptoms, and management

    Complete disability, home health care, FMLA paperwork with FGP medical secretary

    Complete paperwork for prior authorizations, denials/appeals with FGP medical secretary

    Prepare information for peer to peer authorizations

    Keep running list of patients who need follow-up phone calls

    Follow-up hepatology sign out

    Follow patient labs, tests: alert APP or MD if abnormal, call patient if results normal

    Assist in office operations to ensure efficiency and increased access to hepatology patients referred to the health systemAdditional duties as assigned

    Minimum Qualifications:
    To qualify you must have a Licensure: Current registration as a Professional Nurse in New York State
    Education: Bachelors Degree in Nursing
    At least five years of clinical nursing practice, ability to apply nursing process toward achievement of specific outcomes in an organized fashion.
    Self-directed worker, able to work independently; excellent interpersonal, communication and organizational skills; creative problem solving and critical thinking ability, understanding of complex antibody profiles.
    Previous experience in hepatology patient population..

    Required Licenses: Registered Nurse License-NYS

    Preferred Qualifications:
    Previous experience with hepatology population preferred.

    NYU Langone Health provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $88,984.58 - $130,193.65 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
