    RN Case Manager

    Description

    We are looking for an experienced Registered Nurse Case Manager to join our team. The ideal candidate will have a strong background in patient care, medical terminology, and the ability to effectively manage a caseload. The successful candidate will be responsible for developing and implementing patient care plans, coordinating with other healthcare providers, and providing education and support to patients and their families. The Registered Nurse Case Manager must be able to assess patient needs, develop appropriate treatment plans, and provide quality care in a timely manner. Must have Acute Care, In-Patient Case Management experience in current or previous role.

    Responsibilities:

    • Coordinate patient care services with other healthcare providers
    • Develop and implement patient care plans
    • Monitor patient progress and adjust care plans as needed
    • Provide education to patients and families about health conditions and treatments
    • Assess patient needs and develop individualized care plans
    • Document patient care services in medical records
    • Communicate with physicians regarding changes in patient status

    Salary:

    • $60.00 - $63.00 per hour

    Schedule:

    • Monday to Friday 8am-4pm or 9am-5pm
