Lutheran Services Florida (LSF) envisions a world where children are safe, families are strong,and communities are vibrant.
LSF is looking for a talented Case Manager for our Home Study & Post Relief Services program (HS/PRS) who would like to make an impact in the lives of others.
The schedule for this position is 8:30am - 5pm and requires the candidate to be bilingual, preferably Spanish.
Purpose & Impact:
The HS/PRS Case Manageris responsible for provision of case management and supportive services to unaccompanied immigrant children released from federal custody.
Essential Functions:
- Conduct home visits and phone contacts with the dual purpose of assessing safety and appropriateness of childs placement with their caregiver (sponsor) and connecting child to services in the community to address risk factors.
- Ongoing case management with a focus on screening and making appropriate referrals to address childs needs in the following domains: Placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, child enrichment activities.
- Provide crisis intervention services directly, as needed, or connect family to services to ensure minors safety and well-being in a crisis situation.
- Submit detailed and thorough documentation in compliance with LIRS policies and procedures.
- Utilize social work best practices for thorough assessments of childs progress and services provided.
- Maintain regular and timely communication with supervisor around any issues concerning the childs safety, stability, or well-being in the placement.
- Participate in ongoing supervision.
- Other duties as assigned.
Other Functions:
- Attend all organizational required training.
- Attend all departmental and program meetings to ensure that up-to-date information is received and/or information of policy changes or practices are adhered to.
- Attend training that will enhance professional growth in the area of case manager and documentation or other topics as deemed appropriate by the program manager.
- Travel as needed for training, conferences or to transport youth to destinations located within the U.S.
- Maintain a safe, clean, and hazard-free work area.
- Assists Program Manager with Internal Audits and other projects.
Physical Requirements:
- Must be able to pass an FBI background check as well as a CA/N check for every state in which they have resided in the last five years.
- Able to travel to other counties specified in the contract up to 70% of the time.
- Able to sit and stand for long periods of time, utilizes telephone, computer, and typewriter.
- Able to work in a fast-paced environment.
Education:
- Bachelors degree in behavioral sciences, human services, or social services field OR;
- A minimum of 3 years progressive employment experience in the aforementioned fields in addition to a high school diploma (years of advanced education in aforementioned fields can be substituted for years of employment experience)
Experience:
- Child welfare and/or case management experience is strongly encouraged.
- 3-year experience preferred working with refugee or immigrant children.
- Cross-cultural or international experience preferred.
Skills:
- Must be computer literate with working knowledge of Microsoft Windows (Word Perfect, Microsoft Word, Excel)
- Must possess a valid state drivers license and be eligible to drive to facilitate program services as required by contractual agreement.
- Required to work a flexible schedule to facilitate program services.
- Knowledge of community resources and ability to connect families to community services.
- Excellent clinical and crisis intervention skills.
- Excellent verbal and written communication skills.
- Critical assessment and analysis skills.
- Bilingual English/Spanishrequired.
