As a?Licensed Practical Nurse (LPN) Visit Nurse?you will:?

• Provide?in-home?direct patient care according to the?providers?prescribed plan of care (and nursing?scope of practice) while closelymonitoringprogress/recovery?????
• Complete skilled assessments and provideexceptional patient-centered care in the home???
• Ensure an accurateevaluation and implement a comprehensive care plan that addresses patient needs?
• Communicate patient conditions and collaborate with appropriate providersto deliver care when patient needs evolve?
• Complete all clinical documentation in accordance withagency protocol and Medicare/Federal guidelines?
• Provideeducation to patients and family members involved inpatientscare.?
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care?

To qualify?for a?Licensed Practical Nurse (LPN)?position?with us, you will need:????

• Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as anLPN/LVN in the state(s) in which he/she practicesand have atleast1200 hoursof experience as an LPN/LVN or completion of the Office's preceptor program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN)in the state associated with this position?????
• Current CPR/AED/BLS/First Aid certification????
• Reliable transportation to/from care sites and/or work locations?????
• Two (2) years of experience practicing as a Licensed Practical Nurse (LPN in a home health or similar setting
