Social Worker

SUMMARY: Responsible to plan, organize, and implement Social Work / Case Management program. Provide social work services to patients and families, assuring that the medically related emotional and social needs are addressed on an individual basis. Will assist Utilization Review RN with discharge planning and aftercare arrangements. Maintain contact with referring agencies and other community resources. Will coordinate and facilitate swing bed and assisted living activities per regulations.

EDUCATION and/or EXPERIENCE:

• Bachelors degree or Masters degree in social work or human services field (psychology, rehab counseling, etc.) and 1 year supervised social work experience in healthcare setting.
• Dependent Adult and Child Abuse are required within six months of employment.

CERTIFICATES, LICENSES, and/or REGISTRATIONS:

• Child/Adult Abuse certification required within 3 months of hire.
