    Registered Nurse, Cardiac Rehab, Per Diem

    POSITION SUMMARY: The Cardiac Rehab Staff Nurse reports to the MMH Cardiac Rehabilitation Site Manager.# The Registered Nurse supervises medically prescribed exercise therapy appropriate to client#s age, medical and emotional/mental status.# The RN provides education classes, concentrating on client#s understanding of the cardiovascular system and heart-healthy life style modifications necessary to maintaining optimum health.# The Cardiac Nurse is accountable for staffing in all areas of patient services under Cardiac Rehab/Cardiac Stress/TEE and is responsible for marketing of program, orientation of the cardiac patient and participating in Quality Improvement. The Cardiac Nurse will participate in the operational duties of the department, such as but not limited to:# Quality Improvement, Policy and Procedure maintenance, revising and development of billing, gathering of statistical data, developing departmental educational opportunities and participating in the on-going growth and maintenance of the excellent customer services provided to all that ECHN serves. # EDUCATION/CERTIFICATION The Registered Nurse must be a graduate of an accredited professional nursing education program Have a current Connecticut license. Participates in community education by interacting regularly with the community thereby increasing awareness of the prevention and treatment of heart disease.# Actively represents ECHN at rehabilitation related professional organizations. Assesses the patient#s condition and care needs by collecting accurate and thorough data on admission and on an ongoing basis through the performance of cultural and psychosocial assessment, including review of the medical record as per department policy. Exemplary:# Anticipates changes in the patient#s condition, based upon patient assessment data and recognizes subtle abnormalities in the patient#s condition and anticipates the effect on outcomes. 4. Accurately identifies actual and potential nursing diagnoses and/or problems. Exemplary:# Groups and analyzes assessment data to identify specific patient issues or needs with consideration of interdisciplinary issues in the formulation of nursing diagnoses. 5.# Assesses patient and/or family learning needs, including the ability and readiness to learn and# identifies and arranges appropriate resources to assist in the educational process. Exemplary:# Analyzes trends in the patient#s needs, researches literature, contributes to the development of patient education resources. 12.# Consistently prepares and administers medications and IV fluids accurately, according to departmental policies while collaborating with the healthcare team to provide appropriate medication for patient#s condition(s).## Exemplary:# Identifies potential problems in medication administration and makes recommendations for quality improvement measures. 13.# Consistently anticipates emergency situations and takes action to prevent or minimize a negative outcome by demonstrating the ability to organize and delegate appropriately in emergency situations. Exemplary:# Takes appropriate action to improve the emergency response system. 14.# Evaluates the patient#s response to planned interventions and makes appropriate modifications to the plan of care by collaborating and communicating with the other members of the healthcare team regarding changes in the plan of care. Exemplary:# Creatively designs and implements interventions to minimize the patient#s length of stay and maintain positive outcomes.#
