    Patient Care Coordinator, Home Health

    Upon receipt of a referral, thePatient Care Coordinatorconducts home health patient care coordination, determines the patients eligibility/need for services, coordinates care plan development, and assists branch operations in clinical coordination and communication with referral sources. The Patient Care Coordinator works to improve patient care and communication through the care continuum. Works under general direction.

    Essential Functions:

    • Develops/maintains contact with key accounts as assigned. Promotes exemplary customer service to physicians, facility personnel, patient, and patient family at all times
    • Ensures effective communication and collaboration with branch staff and field sales resources via weekly meetings. Maintains market awareness and prepares competitive updates
    • Primarily conducts pre-discharge facility visits at the physicians' request to determine the need/eligibility for home care
    • Assist the branch in the timely processing of physician orders
    • Informs hospital personnel, patient, and/or patient family of case acceptance and documents all demographic, clinical, payer, and other information as required on company-approved forms to support diagnosis and home healthcare orders. Provides input to care plan development and informs the Company location clinical staff (e.g. Intake Specialist, Director Clinical Management, and/or Manager Clinical Practice) of patients acceptance and ensures that patients needs are appropriately matched.
    • Works collaboratively with Hospice Division counterparts as applicable, representing the Companys comprehensive home health and hospice deliverables
    • Abides by all operational compliance with internal controls and reporting
    • Participates in special projects, quarterly Customer Appreciation Week events, and specialty program launches and performs other duties as assigned.
    • Adheres to and participates in Companys mandatory HIPAA privacy program/practices, Business Ethics and Compliance programs/practices, and all Company policies and procedures
    • Abides by and demonstrates the company Mission Vision Values through both job behavior and job performance on a day-to-day basis.

    Required Experience/Skills:

    • Bachelors /Associates degree in Nursing or the equivalent.
    • Current RN or LPN/LVN license, plus a minimum of threeyears of clinical experience
    • Strong knowledge of home health regulations, Medicare requirements, and care coordination skills required.
    • Excellent customer service, organization, time management, problem-solving, and communication skills required
    • Demonstrates the ability to work independently.
    • Microsoft Office proficiency is required.
    • Ability to appropriately represent Company service capabilities to the patients, family, and referral source audience.
    • Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions
    • Registered nurse licensure required to perform patient assessments, secure physician orders, and other responsibilities that require the skills and abilities of a registered nurse per practice
    • Must read, write and speak fluent English.
    • Must have good and regular attendance.
