    Mental Health Clinician Case Mgr - LADC

    JOB DESCRIPTION:

    The Mental Health Clinician Case Manager - LADC position provides direct clinical assessment, case management services and individual/family/group psychotherapy to patients prescribed medication for opioid use disorder (MOUD) in their primary care homes. Included in these services are providing and documenting home health services as defined by Vermont's Blueprint for Health, including comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support and referral to community/social support services.

    The Mental Health Clinician Case Manager - LADC is responsible for the diagnosis of substance use and mental health disorders in accordance with the DSM-5 and ICD-10; formulating the plan of care in collaboration with patient's MOUD prescriber and Spoke RN; consultation with primary care and other referring providers in the community; treatment plan updates; coordination of services with crisis and inpatient psychiatric providers as needed; contact with significant collateral members of the patient's support network; continued care planning; and written documentation of treatment interventions and home health services. The ideal candidate is able to demonstrate professional flexibility and effectively coordinate high acuity patient care with multiple providers within and outside of our health network.

    EDUCATION:

    Applicant must have:

    - Masters degree minimum in mental health field

    - Active licensure in the State of Vermont as an Alcohol and Drug Counselor (LADC), Independent Clinical Social Worker (LICSW) and/or Clinical Mental Health Counselor (LCMHC)

    - Training or certification may be required in a specific area: Case Management, Group Therapy, Cognitive Therapy, Dialectical Behavioral Therapy or Eye Movement Desensitization and Reprocessing.

    EXPERIENCE:

    - At least 3 years direct clinical experience post licensure providing individual/family/group psychotherapy and/or case management services

    - Experience with substance use disorders and co-occurring mental health disorders

    - Familiarity with psychotropic medications, specifically buprenorphine and naltrexone, and their role in treatment

    - Training in acute crisis intervention

    - Training in suicide risk assessment and symptom reduction

    - Strong interpersonal and communication skills are necessary

    Strongly Preferred:

    - Experience with substance use disorders and people on MOUD

    - Familiarity with Vermont's Hub & Spoke system of care

    - Experience working with a multidisciplinary team

    - Training and experience in CBT/DBT approaches to acute mental health symptom reduction or alleviation.

    Additional clinical certifications and skills in specific clinical specialties are welcomed.
