SUMMARY
Licensed Practical Nurse will provide home health services under the direction of a Registered Nurse to the Rochester Regional Certified Home Health Agency (CHHA) clients per Medicare, Medicaid and NYS Department of Health (DOH) regulations. Provide superior customer service by modeling the Brand Promise and Core Values.

ATTRIBUTES
Compassionate, warm and patient focused
Graduate from an accredited School of Practical Nursing
Exceptional documentation skills and professional behavior
One year of nursing experience
RESPONSIBILITIES
Documentation. Completes accurately and timely all documentation of assigned clients care per Federal and State regulatory standards.
Education. Participates in the CHHA Service Education Program. Maintains updated knowledge base of community health practice and community resources.
Rotation. Participates in the holiday/evening/weekendrotation.
Teamwork. Observes and documents signs and symptoms related to patients health status and changes in their condition and reports this to the registered nurse. Obtains the registered nurses assessment and signature on all assigned patients.
Patient Care. Contributes to the identification of basic physical, emotional and cultural needs of the patient through observations and interactions.Provides routine nursing services, information and education to patients during routine care.Collaborates with the appropriate registered nurse to revise care plans when the patients needs are not being met or have changed.

HS (Required)LICENSES / CERTIFICATIONS:
BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

13021The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
