Shift: 8:00 AM - 4:30 PM

Experience:

24 months of Case Management experience is required.

Requirements:

Candidates must have a California license in hand at the time of application.

This role may require floating to additional units and locations

Candidates must live at least 50 miles away from this facility in order to be considered a traveler when applying for this role.

COVID vaccination required for onboarding. Religious and medical declinations accepted.

Flu vaccination required for onboarding. Religious, medical, and personal declinations accepted.

Additional Details:

Must have 2 years acute care with at least 1 of the 2 being LTAC experience.

Shift: Monday - Friday. Weekends on Request

Familiar with LTAC InterQual, knowledgeable with Continuity of Care Record (CCR)
