    If making a positive impact in the lives of others is a constant on your to-do list youll LOVE working with a team that puts people first.Mosaic is looking for aLicensed Practical Nurse (LPN)ORRegistered Nurse (RN)to join our team!In this role, you will provide health care to individuals receiving services in the homes by scheduling medical appointments, assessing condition, reviewing medications, completing any necessary medical forms and by updating medical records.Who will love this job:

    What YOULL Do:
    • Coordinate physician and health care services for people served
    • Conduct health care assessments and write health care plans
    • Facility training and orientation of new staff regarding healthcare procedures
    • Provide on call assistance to staff as needed

    SCHEDULE:

    Nights11pm-7aevery other weekend, rotating holidays

    Evenings 3-11pmevery other weekend, rotating holidays

    PRN/ONCALL- would need to be available evenings and nights -Will require holiday and weekend rotation - 24 hours a month.

    What YOULL Need:

    • State license required in nursing
    • 2 years of general nursing experience NEW GRADUATES WELCOME!
    • Must be able to lift, push and pull at least 50 pounds.
    • Valid Driver's license
