BMT Long Term Follow Up Nurse Coordinator

What you will do in this role:
- Coordinate care of the post-transplant patient in the outpatient setting.
- Monitors patient/family after transplantation for physical, emotional, psychosocial, and financial needs and intervene when appropriate.
- Responsible for long-term follow-up, which includes but is not limited to: periodic updates on patient condition, coordinating follow-up visits with Blood and Marrow Transplant physician, scheduling disease restaging (and other tests as needed) services.
- Maintain awareness of outstanding lab/imaging results and relay to provider for prompt follow up.
- Coordinate referrals to physical therapy, home health, and other specialties as needed.
- Coordinate survivorship efforts between transplant team and outside physicians, optimizing patient recovery and health maintenance after transplant.
Qualifications:
- Valid TN RN license
- 3-5 years oncology experience including stem cell transplant and or heme-malignancy, adult setting preferred
- Current BLS
- ONS Chemotherapy Certification preferred
- BMTCN required within 4 years of hire
- BSN or Masters degree preferred
- Apheresis experience, preferred

1. Assists in the review, revision, development, implementation, coordination and integration of policies and procedures in accordance with all government and institution regulations.
2. Identifies/coordinates/participates/monitors the implementation of corrective action plans for deficiencies identified through utilization review clinical record audits, client satisfaction surveys, TJC/, Foundation for the Accreditation of Cellular Therapy (FACT)/ ACoS / FDA/ CAP and other state or federal surveys, and Performance Improvement Plan.
3. Ensures appropriate documentation of activities developed, implemented and maintained for the Quality Management Program.
4. Coordinates/presents Annual Quality Assurance Report /Program Evaluation.
5. Coordinates/implements/participates/monitors patient safety programs, including infection control, hazardous communication, security, equipment management and inspection and safety audits across all areas of the SCCBC.
6. Creates and maintains a collaborative working environment with program physicians, program administration, clinical, collection, and processing staff to ensure overall program success.
7. Develops relationships, collaborating with hospital or community entities that support and promote SCTCTP
