RN Stroke Coordinator
A 250 bed San Francisco Bay Area non profit Medical Center is seeking a Registered Nurse Stroke Coordinator for their growing Neuro Program.
The Stoke Coordinator will be responsible for all oversight and coordination of the Stroke Program, and will collaborate with the Nurse Director Medical Surgical Services and the Medical Director of the Stroke Center.
The Medical Center is known for their Neuroscience Department, and has been constantly improving the quality of care. The Center treats patients suffering from a number of conditions such as tumors, cerebrovascular disorders, acute and chronic pain syndromes, and peripheral nerve disorders.
The Nurse Stroke Coordinator RNwill be responsible for coordinating Joint Commissionaccreditation toachieve Primary Stroke Center Certification. This includes the collection, compilation and analysis of dated related to the diagnosis of stroke, toprepare summary reports of improvement activities. Nurse Stroke Coordinator RN will make daily roundsto ensurecompliance with indicators, and education of clinical and medical staff related to clinical outcomes.
The Stroke Center Coordinator will be responsible for the coordination and oversight of the Stroke Program. This individual will also be accountable for assisting in community screening/education events, follow-up during hospitalization, patient teaching and post-hospitalization follow-up. The Stroke Program Team Lead is responsible for outcome measurement for selected populations and monitoring of patient/family satisfaction. The hospital isa Joint Commission designated Comprehensive Stroke Center andearned the American Stroke Associations Heart-Check mark for Advanced Certification for Comprehensive Stroke Centers.
