A Health System supported hospital located North of Sacramento is seeking a Case Management Manager to join their care team dedicated to providing high quality care to every patient that walks through their doors.

The Case Management Manager will join a 230+-bed acute care hospital that values its commitment to excellence in quality and safety and provides patients and the community with quality and safe care. This full-service facility offers a wide array of specialty services including surgical OR, emergency services ER, cardiology, womens services OB, and orthopedics. This organization is fully accredited by the Joint Commission.

Reporting to the Director of Case Management, the Case Management Manager will oversee all departmental activities within case management. The Case Management Manager will work collaboratively with leadership to functional excellence and completion of operational objectives. They will serve as a mentor by providing leadership to staff. The Case Management Manager is supported by a team of Physicians, Advanced Practice Providers, Registered Nurses, and Social Workers.

Requirements include:

• Minimum BSN or MSW
• 5+ years of Hospital Case Management experience
• 1+ year of Leadership experience
• Ability to lead and communicate effectively
