    RN Palliative Care

    About the unit

    Corewell Health's Palliative Care team offers comprehensive, multi-disciplinary care in multiple care settings. Our experts incorporate their knowledge from different areas, work together to conduct a thorough assessment, and implement an individualized treatment plan for the patient/family that helps address their symptoms and goals of care related to a serious or life limiting illness. The team-based approach ensures an optimal, whole-person treatment plan is developed and implemented for the patient.

    About Outpatient Clinics

    Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.

    Scope of Work

    The Palliative Care Nurse acts as a coordinator, expert, and educator for Palliative care patients and their family members. They will collaborate with Physicians, Advanced Practice Providers, Social Workers, Chaplains, and other members of the health care team to achieve high-quality patient outcomes. They will be responsible for clinical triage, coordination of care, patient education and teaching. The position will primarily work on site at Lemmon Holton Cancer Pavilion with some remote work available.

    Qualifications

    • Associate's Degree Nursing or Equivalent Required
    • Bachelor of Nursing (BSN) Preferred
    • MI Registered Nurse (RN) Required
    • AHA or ARC Basic Life Support (BLS) Required within 90 days
    • 1 year of experience as an RN Preferred
    • Experience with Hospice, Palliative Care, End of Life, or Oncology highly Preferred
