A renowned Bay Area Hospital is seeking an experienced Nurse Manager of Case Management to oversee our team of case management professionals. This 300+ bed hospital is part of a large and stable healthcare organization that is known for delivering the highest standard of care to Bay Area patients.

In this role, you will be responsible for supervising and supporting case managers, developing patient care plans, coordinating discharge planning, and improving healthcare delivery.

We are looking for someone with a Bachelor's degree in Nursing and 3-5 years of leadership experience in case management in a hospital setting. Excellent leadership abilities are essential, as you will mentor case managers, facilitate team meetings, and ensure department protocols are followed. Superior communication and organization skills are required to effectively interact with patients, families, insurance companies, and other healthcare providers.

Key Responsibilities:

• Recruit, hire, train, and supervise case management staff
• Oversee patient admissions, continued stay reviews, and discharge planning
• Create comprehensive care treatment plans and arrange services for uninsured/underinsured patients
• Serve as a liaison between case management and other departments
• Track performance metrics such as length of stay, 30-day readmissions, and more
• Identify areas for improvement in current case management workflows
