    Nurse Supervisor LVN-LPN

    Valle Verde in Santa Barbara, a HumanGood community recently named 'Best CCRC in the U.S. 2024' by Newsweek magazine, is hiring an Nurse Supervisor - LVN for its Skilled Nursing team.

    Salary Range: $85,000 - $105,000/year Depending on Experience

    Under limited supervision, the Nurse Supervisor LVN is responsible for the nursing needs of residents, while actively incorporating our Philosophy for Person-Directed Care. S/he ensures a safe and healthy environment for residents, staff, and visitors to participate in the residents daily life rhythm; supervises and coordinates nursing services; trains team members accountable for the work units adherence to established policies and procedures.

    Additional essential duties and responsibilities include, but are not limited to:

    • Supervising and leading CNA and LVN team members, ensuring all procedures are followed during emergency situations.
    • Assuring the nursing staff is carrying out their respective duties toward resident care in a caring and timely manner.
    • Actively engaging residents in participating in daily activities which are part of residents rhythm of daily life (including desired hygiene, grooming, and mobility).
    • Assessing residents' needs at the time of admission, and continually evaluating residents condition during the duration of their stay.
    • Making frequent rounds and utilizing the nursing process to identify resident conditions that require further evaluation and treatment by a physician.
    • Reporting significant medical changes to the relevant team members, Physician, DON, HSA and designated family members as appropriate.
    • Coordinating the residents plan of care with the interdisciplinary team (IDT), physicians, medical personnel, and residents family/responsible party.
    • Interacting with other departments such as laboratory, radiology, dietary, facilities, information technology, pharmacy, social services, activities, etc.

    To be successful in this role you would have:

    • Completion of an accredited LVN/LPN Nursing Program and current unencumbered LVN License
    • Current CPR Certification
    • Minimum of 1 year of professional supervisory or management experience
    • Current/prior supervisor experience in a skilled nursing, post-acute or long-term care environments highly desirable
    • AS or BS Degree, preferred
    • Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members
