    Position Summary:
    We have an exciting opportunity to join our team as a Supervisor, Home care- Nassau/suffolk.

    In this role, the successful candidate Works collaboratively with the Administrator/Director of Patient Services and is responsible and accountable for establishing and maintaining standards, the delivery of patient care, directing and facilitating work performance of all personnel in the agency and maximizes patient outcomes. Serves as a patient care advocate and role model in the performance of direct and indirect patient care and assumes leadership role and function in absence of the Director.

    Job Responsibilities:

      • Performs other duties as assigned
      • Assists Director in establishing, monitoring and evaluating standards of patient care focusing on measurable outcomes. Consistently addresses patient care needs in a timely manner. Screen inquiries and referrals and processes community requests for Home Care service in accordance with Agency policies and procedures. Participates andor sponsors multidisciplinary process improvement activities for program specific and hospital wide initiatives. Assures compliance with Joint Commission, Department of Health, NYS Department of Social Services and other regulatory agencies, as well as agency policies and procedures. Confers reasonable budget projections for staff and supplies and effectively manages fiscal responsibilities.
      • Assists with attainment of specific goals and objectives in accordance with organizational and agency strategic goals. Interprets, implements and promotes agency policy; recommends policy modifications. Employs, orients, trains, supervises and appraises the performance of nursing staff; and other staff as assigned; provides case consultation and serves as a role model for nursing practice. Oversees nursing staff's work schedules, which provide for twenty-four (24) hour, seven (7) day coverage.
      • Assesses the health care of patientsfamilies and responses to care rendered, and progress. Assesses, evaluates and documents patients' responses in collaboration with nursing and interdisciplinary team interventions.
      • Collaborates on assessment of patient's clinical status with team members and attempts satisfactory resolution of patient clinical issues in a timely manner. Addresses patientteam member issues following established communication hierarchy. Participates in identification, investigation and resolution of actual andor potential risk managementquality careethical issues. Holds team members accountable.
      • Assumes leadership role and functions in the absence of Director. Ensures the provision of age appropriate and safe careservice (population served). Maintain clinical knowledge base appropriate to patient population.
      • Ensures flow of information to optimize operations. Encourages a functional, safe and therapeutic environment for patients and staff through regular orientation and education of staff on safety issues. Ensures staff compliance with Infection Control techniques both in practice and attendance at annual Infection Control In-service. Ensures a well-functioning clean environment for staff. Performs patient home visits as indicated.
      • Responds in a timely manner to organizational information requests for reports, and data. Review 485's from laptop clinicians and release for printing within 24 hours of receipt 100% of the time via computer. Consistently utilizes computer to communicate, document, track and evaluate information.

    Minimum Qualifications:
    To qualify you must have a Bachelor's Degree Nursing. Current and valid NYS Registered Nurse License. 3-5 years in Clinical Nursing. 1-2 years in Community Health nursing. Excellent communication (both oral and written), interpersonal, organization, leadership skills. Basic computer skills.

    Preferred Qualifications:
    Master's Degree Nursing,Health Education, Community Health, or Public Administration. 9-11 years Nursing (at least one year in Homecare,Hospice or LTHHCP setting).

    Qualified candidates must be able to effectively communicate with all levels of the organization.

    NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $105,999.99 - $128,393.26 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
