Overview:We're looking for a Patient Care Manager to join our team.
You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager. You will be responsible for the day to day operations of your assigned home health site, administering the clinical aspects of the home health program and for ensuring the provision or quality of care to patients.This is an exciting opportunity to share your clinical expertise, passion for home health and be part of a team that strives for outstanding outcomes in the home.This role will involve working full-time weekdays in our office in DePere and being a part of an on-call Administrator rotation of every 3rd week/weekend. Thisrole will also involve providing visits in the community as needed.
- Managing the site in the absence of the Executive Director, Administrator, or Senior/Executive Patient Care Manager.
- Managing and overseeing clinical and administrative and recordkeeping functions
- Responding to phone referral/inquiries in a timely, professional, and compassionate manner
- Participate in on-call nursing schedule and providing direct skilled bedside nursing care in patients homes
- Bachelor degree in nursing from an accredited school of nursing. Unless superseded by requirements listed in the state-specific section
- Minimum two years full-time clinical experience under qualified supervision, one of which shall be in a health care institution and one of which shall be in a hospice or home health care agency or a related community health program. Unless superseded by requirements listed in the state-specific section
- Must be able to effectively operate nursing assessment tools and office equipment to include, computer and or tablet
- Sensitivity to feeling of the terminally ill and their families before and after a patient's death
- Knowledge of terminally ill patients and their families along with an understanding of the hospice concept
