Job Summary and Qualifications: RN Clinical Rehab Liaison PRN
- Position is responsible for developing referral relationships with hospital social workers, case managers and physicians. Liaison reviews St. Marks Hospitals list of potential acute rehabilitation patients, identifies appropriate patients for admission and follows-up with the patients social worker/case manager.
- Liaison completes the mandatory Pre-Admission Screening tool for potential admissions and reviews with the rehabilitation physician for final approval.
- After rehab physician approves patient for admission to the Acute Rehabilitation Center, the liaison submits all relevant information to the patients insurance company in order to obtain pre-authorization for admission.
- Position is responsible for marketing/referral relationship development within St. Marks Hospital as well as marketing to other hospitals, skilled nursing facilities and home health agencies in the community.
- Prior experience in post-acute healthcare marketing and census development is preferable. Applicant must possess strong interpersonal skills in face-to-face interactions and on the telephone.
What qualifications you will need:
- Current Utah State RN license in good standing.
- Graduate of an accredited school of nursing.BSN preferred.
- American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification preferred. Must obtain BLS or BCLS within 30 days of hire and maintain certification.
- American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification preferred. Must obtain ALS or ACLS within 6 months of hire and maintain certification.
- Minimum of 2-years clinical experience with strong clinical expertise and interpersonal skills demonstrated. Acute rehabilitation experience is preferred.
