    Community Care RN

    Schedule: 4x8s, Days, M-F (open to 5x8s)



    **This position requires 90% at home/in facility visits, and 10% in home clinic**



    A health care facility in the Portland, OR area is looking to hire a RN who will be responsible for caring for elderly patients. This nurse will be working alongside members of the care team (including medical assistants, LVNs, CNAs, etc.) to provide nursing care that promotes optimal health and independence. They will also be performing nursing assessments and using critical thinking skills to ensure the highest level of care for their patients. This role requires the RN to have their own personal vehicle, as they will be making episodic visits into patients' homes. Mileage is reimbursed.

    Home health experience
    -2+ years as a RN, ideally working with mostly geriatric patients

    -ASN or BSN with valid OR State nursing license

    -RN license in the state of Oregon

    -BLS certification (upon hire)

    -Personal vehicle and ability to travel in the community to see patients
