Job Summary and Qualifications
As a Palliative Care Nurse Coordinator, you will be responsible for the development and implementation of a palliative care program. You will provide guidance, support, and education to healthcare team members in the overall delivery of services related to palliative care.
- You will establish a palliative care plan for all palliative care patients based upon assessments, patient care standards, and patient and family preferences
- You will consult and collaborate with healthcare team members to develop a plan of care and palliative care recommendations
- You will ensure consistent, complete, and thorough nursing documentation
- You will review physician orders related to palliative care, and write or enter orders into the system accurately
- You will provide timely communication to nurses and/or physicians regarding changes to the plan of care and/or changes in the patients condition
- If necessary, you will provide basic life support in collaboration with the healthcare team
- You will maintain and review patient records, charts, and documents for accuracy, quality, and best practices related to palliative care
- You will provide palliative care data, including the number of consults, referrals, conversions to hospice, and changes in level of care
Job Requirements
Experience
- 1 year of clinical nursing experience as a Registered Nurse (RN)
- Previous experience in Hospice and Palliative Care nursing
- Critical care experience in a hospital setting preferred
Education
- Associates degree in Nursing (ADN)
- Bachelor of Science in Nursing (BSN) preferred
- Advanced Practice Nurse (APN) preferred
Licenses and Certifications
- Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License, Texas license must be obtained within 90 days of hire
- Basic Life Support (BLS) certification as per St. Davids Healthcare policy
- Certified Palliative and Hospice Care Nurse (CHPN) preferred
