    PRIMARY PURPOSE

    Nurse Home Visitors provide high quality nursing care to patients in their home. The RN is a licensedprofessional who uses the Parkland nursing professional practice model to coordinate patient caredelivery, that focuses on a particular patient population(s)/specific health condition, by the health careteam. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based onresponses to health problems, develops and implements an individualized plan of care, and evaluates thepatient's response. The RN utilizes knowledge of patient needs and the healthcare environment to assistpatients to transition through the healthcare encounter without any preventable complications or delays.The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, eachpatient's condition, and the competencies of the employee.

    MINIMUM SPECIFICATIONS

    Education:

    -Must have a Bachelor's degree in Nursing.

    Experience:

    -Must have one to two year's experience in maternal/child health, public health or home visitation.

    Certification/Registration/Licensure:

    Cost Centers: 74030 Nurse Family Partnership, 84730 NFP-PCHP, 88550 Post Natal NHV, Program,60620 CHNA-eMCAP
    Must have current RN license or valid temporary permit with the Texas Board of Nursing or validCompact RN license in good standing without any stipulations.
    Must have current healthcare provider BLS for Healthcare Providers certification from one of thefollowing: American Heart Association;American Red Cross; orMilitary Training Network
    Must have a Texas Class C Driver License
    Upon hire and every 2 years complete an Automatic Background Check through TexasDepartment of Family and Protective Services
    May be required to have additional specialty certifications

    Skills or Special Abilities:

    Provides care to assigned patient population in accordance with the current State of Texas NursingPractice Act, ANA Scope and Standards established protocols, multidisciplinary plan of care, andclinical area specific standards.
    Must be able to communicate and collaborate effectively with a diverse group of patients, families andhealthcare staff.
    Must be able to demonstrate a working knowledge of specific patient populations, and be able todemonstrate knowledge of disease processes affecting this group.
    Must be able to demonstrate a working knowledge of PC operations and the ability to use wordprocessing software in a Windows environment.
    Must be self-directed and capable of priority setting and problem solving.
    Must be able to demonstrate patient centered/patient valued behaviors.
    Must have reliable vehicle transportation for home visits.
    Must be able to maintain a driving record that meets the criteria established by Parkland procedures.
    Must be familiar with traffic regulations and locations in the surrounding areas.

    Job Accountabilities
    1. Identifies ways to improve work processes and improve customer satisfaction. Makesrecommendations to supervisor, implements, and monitors results as appropriate in support of theoverall goals of the department and Parkland.
    2. Stays abreast of the latest developments, advancements, and trends in the field by attendingseminars/workshops, reading professional journals, actively participating in professionalorganizations, and/or maintaining certification or licensure. Integrates knowledge gained intocurrent work practices.
    3. Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact thearea. Develops effective internal controls designed to promote adherence with applicable laws,accreditation agency requirements, and federal, state, and private health plans. Seeks advice andguidance as needed to ensure proper understanding.
