Shift: 8:00 AM - 4:30 PM

Experience:

24 months of role experience is required.

12 months of Case Management experience is required.

3 months of experience with Meditech is required.

Requirements:

Candidates must have a Florida license or compact license in hand at the time of application.

This role may require floating to additional units and locations

Candidates must live at least 50 miles away from this facility in order to be eligible to apply for this role.

Additional Details:

MIDAS is the case management documentation system. NAVI is the electronic referral system.

Scheduling Requirements: Monday through Friday with rotate into weekends.

Floating policy: May need to float to other units.

Must have: Acute care case management experience.

Must know about multidisciplinary rounds, must know discharge planning and what is required to meet LTACH, SNF, HHC level of care to give appropriate referrals.

Must have Interqual knowledge, be highly organized, and skilled at using the computer.

Dress Code: Business casual, must wear white lab jacket, must wear closed toe shoes. May wear navy blue scrubs with white lab jacket wntil March 1. At that time the dress code changes.

Maximum days off: 10 days
