In collaboration with the Director of Case Management and Manager of Care Coordination, theCareCoordination Registered Nurse will be responsible for collaborating withhealthcareproviders and physicians for problem solving, prioritizing and facilitating patient care objectives, the coordination and development of staff education, and discharge patient education and follow-up for the area assigned.

The Case Management Registered Nursewill be responsible and accountable for unit systems functions, staff performance, and patient care outcomes. TheCare Coordination Registered Nurse will assist Director and Manager of Case Management in accomplishing unit and organizational goals.
