Team Select Home Care is looking for Licensed Practical Nurses to join our Team Select family. Our nurses will document and provide skilled nursing care to our patients, following the developed plan of care and the patients physician orders for each of our patients; while following confidentiality and professional boundaries.Will also prepare clinical and progress notes for our patients, assist the physician, the intermediate care provider and RN in performing specialized procedures.

• Conduct in-home patient assessments and develop care plans
• Administer medications and treatments as prescribed
• Monitor patient progress and adjust care plans accordingly
• Collaborate with the interdisciplinary team to ensure comprehensive patient care
• Maintain accurate and up-to-date patient records
• Perform other tasks as assigned.

Pay range $27 - $32 / hour

• Active LPN License in good Standing
• Previous experience in Home Health Care (preferred).
• Strong communication and interpersonal skills
• Access to a personal device or tablet with Wi-Fi or cellular data connectivity for clinical documentation.
• Maintain current CPR certification.
