    Job Summary

    Care Management is a collaborative practice model including patients, nurses, social workers, physicians, healthcare team members, caregivers and the community. The Care Management process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of the Care Management Team include the achievement of optimal health, access to care and appropriate utilization of resources balanced with the patients right to self-determination.

    Identifies psychosocial issues as they relate to the cancer diagnosis of the patient and family. Provides direct care and consultation to patients, families, and the Health Care Team. This includes counseling and the seeking of resources to answer unmet needs. Also, works closely with the Cancer Care Fund of Litchfield Hills . The oncology social worker also provides consultation services and in-service education programs related to psychosocial aspects of healthcare to their professional staff members in the Healthcare Team. Assists in the development of policies and procedures for the provision of oncology services.

    Incumbents of this position are professional Licensed Clinical Social Workers or MSWs that utilize their education, experience, medical social work skills and the Social Work Process to determine and facilitate the most appropriate level of care and identification of discharge needs. This person has diverse responsibility related to the patient assessment and planning process that includes but not limited to psychosocial, financial and discharge planning. Collaborates with physician to ascertain medical treatment plan and with nursing, other health care team members and health plans to fulfill the treatment plan in the highest quality, cost effective manner. Responsible for ensuring customer satisfaction (patient, family, physician, others), maintaining patient confidentiality and being sensitive to the age and cultural specific patient needs for comfort, privacy and generalized care.

    Qualifications

    • Master's Degree required
    • RN, LCSW, LMFT, LPC, LADC, or Psychologiest licensed required
    • Clinical experience required
    • Experience in a managed care or triage setting preferred
    • Strong Communication and Interpersonal Skills
    • Knowledge of Social Work Process
    • Ability to manage multiple priorities and projects form initiation to completion within prescribed schedules and utilization of resources
    • Ability to effectively utilize a variety of Microsoft Office, Outlook and Meditech HCIS software, Morrisey and Web based programs required.
    • Skilled in Motivational Interviewing
