    • Current New York State Registered Nursing License/Registration
    • Baccalaureate in Nursing (BSN)
    • A minimum of five years' experience in Case Management in an acute care setting
    • Certification in Case Management (CCM) preferred
    • Advanced proficiency in Microsoft Office Suite and case management software
    • Exceptional leadership, organization, and decision-making skills
