    Mercy HaH is an advanced home-based care model, that provides patients with acute care at home that is equivalent to traditional hospital-level care.

    Patients presenting to the Emergency Department with acute care needs that would normally require hospitalization may be 'admitted' to the Mercy HaH program. After receiving medically appropriate transportation home, Mercy HaH patients receive all acute care services in their home, including twice daily nursing or paramedic visits, a clinical provider visits daily (virtually or in-person), medication management, necessary equipment, and 24/7 medical oversight. HaH episodes of care typically last 3-5 days before the patient is 'discharged' back to the community setting.

    Overview:

    The Mercy HaH Patient Associate (LPN) provides day-to-day hospital-based operations, coordination, and logistics support to the HaH clinical team. The Patient Associate (LPN) is a key member of the HaH operations team and is the 'glue' supporting all hospital-based logistics prior to HaH admission. The LPN is a high-energy, patient-facing team member who coordinates and supports an exceptional patient/family experience during the HaH admission process.

    Qualifications
    High school diploma or GED equivalent: Graduate of accredited nursing program
    Responsible for maintaining current LPN license within hiring state with nursing duties performed and must meet all State Board License requirements
    1+ years' experience with patient access and delivery systems, and/or prior healthcare clinical operations experience strongly preferred.
    Computer skills: proficiency with Microsoft Office and Teams; more broadly computer savvy with a demonstrated ability to learn and navigate multiple systems.

    • This position is full-time facility-based role as part of the Mercy HaH program. You must be authorized to work in the United States. Current positions are available in St. Louis, MO (Mercy STL and Mercy South).
