Home Care Start of Care Specialist

Position Summary: Responsible for initiating home care services for newly referred clients. Additionally, the Start of Care Specialist will match appropriate home care aides with client’s to promote independent living at home.

Hours: Full Time Monday through Friday 8 am - 5 pm

Location: 2333-2341 W. 95th Street Bldg. C, Suite 2 Chicago IL

Essential Duties:

• Initiates Home Care Services for new clients in accordance with established payor timelines
• Facilitates communication between new clients, home care aides, case managers and the branch
• Completes and coordinates client/employee schedules in accordance with the client’s plan of care while remaining within authorized service hours
• Communicates changes in service needs to referral agency staff and branch management
• Schedules preferred workers/family caregivers for pre-employment training
• Resolves consumer complaints within the scope of authority
• Notifies branch management of significant events related to service delivery
• Notifies Service Coordinators of new clients received within assigned service area
• Performs other duties as assigned

Position Requirements & Competencies:

• High school diploma or equivalent
• Data Entry
• Customer Service
• Case Management
• Computer proficiency - MS Office
• Occasional travel between facilities may be required
• Must have reliable transportation, valid driver license and state required insurance
