Position Summary: The Community Outreach Coordinator is responsible for overseeing client referrals, scheduling and performing assessments and marketing to referral sources. This position is also responsible for overseeing VA and Private Duty client referrals. In addition, the Outreach Coordinator assists Veterans and families on navigating the VA system and Long Term Care Insurance. This position will also perform internal/external marketing and complete weekly statistical reports pertaining to intake and discharges.

Essential Duties:
- Responsible for overseeing client referrals
- Follows state and company policies and procedures regarding processing all incoming referrals within given timeframes
- Communicates with clients, caseworkers’/service coordinator, practitioner’ offices, and regional nurses to facilitate client intake and initiation of services
- Obtain necessary documentation for procedure of referrals, including making every effort to obtain the practitioner’s orders within timeframe guidelines
- Coordinate client services between referral source and the company
- Maintain all intake
- Complete required documentation and comply with productivity standards
- Familiarity with accreditations and licensure standards
- Knowledge of the electronic health care record
- Maintains a high degree of confidentiality at all times due to access to sensitive information
- Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
- Follows all Medicare, Medicaid, and HIPAA regulations and requirements
- Abides by all regulations, policies, procedures and standards
- Performs other duties as assigned
Position Requirements & Competencies:
- Bachelor’s degree in Business, Social Services or a related field
- 2-4 years experience in sales, marketing, customer service preferred
- Prior experience in healthcare
- Excellent verbal, writing, and time management skills
- Proficiency in Microsoft Office (specifically Microsoft PowerPoint, Excel, and Word)
- Detail-oriented, organized and able to work independently
- Must have own reliable transportation, a valid driver's license and current insurance by law
