Ambercare Hospice is seeking a detail-oriented, compassionate Community Outreach Administrative Coordinator to support our hospice and home health operations across Las Vegas, Española, and Santa Fe, NM. This newly shaped role blends essential administrative responsibilities with community-based coordination—and offers a clear growth path into a Home health/Hospice Sales Liaison position for those who excel.

What You’ll Do:
Administrative & Admissions Support
- Prepare, submit, and track Admission Notices, Death Notices, and other required documentation
- Assist with referral intake, including gathering information from hospitals, facilities, and community partners
- Coordinate and schedule initial nursing assessments and admissions
- Maintain accurate and up-to-date admission packets, forms, and compliance documents
- Support completion of admission paperwork with patients, families, and facility staff
- Maintain organized, accurate records and documentation
Community & Facility Coordination
- Travel occasionally between Las Vegas, Española, and Santa Fe to support facility partners and internal teams
- Set up and coordinate education events, in-services, and informational sessions at partner facilities
- Assist with outreach efforts to strengthen relationships with referral sources
- Provide clear explanations of hospice services to patients, families, and community contacts
- Help identify alternative services when a patient is not eligible for hospice
Qualifications:
- At least one (1) year experience in homecare, home health, hospice, and/or healthcare-related field preferred
- Prior community outreach experience is a plus
- Strong administrative and organizational skills, with the ability to manage multiple time-sensitive tasks
- Comfortable traveling regularly between Las Vegas, Española, and Santa Fe
- Professional communication skills, including the ability to speak confidently with facility staff, patients, families, and referral partner
- Self-motivated, dependable, and able to work independently while also collaborating with a team
- Comfortable presenting information or assisting with education events, in-services, or facility trainings
- Ability to complete necessary documentation appropriately, legibly, and timely
- Ability to handle stressful and delicate situations
- Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
- Valid driver’s license and proof of insurance are required
