Avera is currently seeking Licensed Practical Nurses (LPN) looking to deliver the best possible nursing care in Avera’s patient-focused settings.

Job Summary
- Conduct daily rounds and communicate to appropriate team members during shift.
- Contacts physician, establishes good working relationship and initiates ongoing information exchange.
- Provide professional nursing care to residents, flexible to patient census and staffing needs.
- Administer professional services.
- Use restraints when necessary and in accordance with established policies and procedures.
- Administer routine medication including IV meds, charts and makes decisions about the use of PRN medications.
- Obtain specimens for lab tests as ordered.
- Take and record TPRs, blood pressures, etc.
- Monitor seriously ill residents as necessary.
- Notify the resident’s attending physician and next-of-kin when any changes in resident’s condition.
- Consistently & thoroughly completes documentation in a timely manner, including initialing medication & treatment admin. records.
- Formulates interdisciplinary plan of care.
- Coordinates resident care with interdisciplinary team.
- Participates in interdisciplinary team meetings.
- Completes/supervises medical record.
- Review resident care plans.
Licensure/Certification/Registration and Experience
- Graduate from an accredited school of nursing with current licensure as a Licensed Practical Nurse in the state of SD.
- Participates in educational conferences and updates and maintains current professional clinical competency related areas of responsibilities.
- BLS must be complete within 6 months of start date and maintained.
- Attends all mandatory departmental meetings and in-services.

Pay
The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
- $25.25 - $36.50 per hour
