A growing home care company with a mission to help veterans age in-home is searching for a *Part-Time Registered Nurse (RN) Case Manager. *
(Our company is not associated with other BrightStar Care companies and or offices. We are independently owned and operated.)
This position offers work-life balance as the company offers flexible scheduling. An individual won't have to work on-call, no evenings and no weekends! Make your own schedule during business hours! *Must be able to commit to a minimum of 24 hours/3 days per week (Fridays are required, and you can choose your other two days to work).*
This is a traveling position with high autonomy! *Get your patient visits done in the morning and work the rest of your day from home if you want!*
* Base Pay: $39-$41/hr

We're looking to speak with applicants that are located within our primary service area. The position will have a focus in working within our primary service area and help cover in our other service areas only as needed.
*Primary Service Areas: *Plymouth, Maple Grove, St. Michael, Rogers, Crystal, New Hope, St. Louis Park, Hopkins, Wayzata, Maple Plain, Mounds, and Minnetonka.
*Other Services Areas:* Rockford, Delano and Buffalo.
*Registered Nurse Case Manager Requirements:*
* Must be Active on the MN Board of Nursing
* Must be current on CPR Certification
* Minimum of one year of experience as a Nurse, _preferred but not required_
* MUST BE ABLE TO TRAVEL
* Must have Driver’s License with insured, reliable transportation
*Registered Nurse Case Manager Essential Duties:*
* Provide in-home care for clients based on personalized plan of care
* Perform caregiver supervisory visits and competency testing
* Perform and maintain on-going client case management
* Thoroughly document assessments and cares using eCharting system
* Provide care coordination for individual clients, including coordination with other agencies, healthcare providers, pharmacies, medical supply companies, etc
* Manage the client’s nursing schedule; be prepared to fill shifts urgently if needed
* Cover for two other case managers when they go on vacation
* Other duties assigned or delegated by DON, including computer work in the office if necessary
