Responsibilities:

Provide individual and family counseling for patients and families
Conduct psychosocial assessment
Formulate and implement the intervention plan
Provide crisis intervention for patients who are in crisis
Provide intensive case management/care coordination service for patients with complex bio-psychosocial needs
Provide advocacy and referral service for patients
Supervise interns and volunteers
Complete documentations and reports
Attend case conferences, agency meetings and staff meetings
Conduct group/workshop for patients and the community
Participate in community outreach and marketing activities
Perform other duties as assigned

Minimum Qualifications:

Master's Degree in Social Work from an accredited social work school
New York State LMSW/LCSW License required
Basic CPR (BCLS) Certification required (Health Center will provide if needed)
Working experience in social services field preferred
Familiar with the Asian-American communities
Good verbal and written English skills
Bilingual in English/Chinese required

Compensation and Benefits:

Salary: $65,000.00 - 68,000.00 per year
