Position Summary:?Primary responsibilities include monitoring and maintaining compliance for compounding and repackaging of medications for the organization; contributing to pharmacy department quality and process improvement initiatives; and identifying opportunities in the medication use process. This position collaborates with health care professionals, staff, and leadership in all settings to comply with pharmacy compounding and repackaging standards, laws, and practices. This role includes responsibility for readiness related to all compounding and repackaging, quality and safety considerations, influencing practice change, optimizing medication use technology, research, and education.

Job-Specific Duties and Responsibilities: 

· Provide subject matter expertise for compounding as it relates to USP and TSBP compliance 

· Identify and prioritize opportunity areas for improving efficiency in compounding and repackaging of medications for all pharmacy practice settings 

· Implement standardized processes for compounding and repackaging of medications 

· Assist in creating formulas for compounded dosage forms not commercially available for pediatric patients, based on clinical evidence and primary literature 

· Develop criteria for and documentation of actions/changes taken/implemented to be compliant with all regulations relating to compounding and repackaging. This includes plans for follow up audits with documentation to ensure changes implemented have the desired positive outcome, improve medication safety, and changes are being sustained 

· Engage staff and managers in creating a culture to maintain and continually improve readiness for compounding and repackaging 

· Collaborate with all disciplines outside of pharmacy as it relates to compounding and repackaging.  

Experience: 

·?Completion of a PGY1 pharmacy practice residency or 3 years hospital experience. Required 

·?Completion of a PGY2 residency in Medication Safety or 2 years of medication safety experience. Preferred? 

Education: 

· Bachelor's degree or equivalent experience Degree (B.S. or PharmD) from an accredited school of pharmacy. Required  

Licenses and Certifications: 

· Current Texas Pharmacist license - Required 

· Compounded Sterile Preparations Pharmacy Specialty Certification (BCSCP) - Preferred
