Description

  • Monitors and assumes responsibility for efficient use of resources.

  • Schedules personal time off such that necessary services (Utilization Review, Discharge Planning, Social Services, etc.) are adequately available.

  • Assists in orientation, training and education to insure that all department members are able to perform as described in their appropriate job description.

  • Uses computer, copy/fax machine, and other office equipment in a safe and appropriate manner.

  • Assists in maintaining files and documents as needed to meet hospital needs and requirements of regulating agencies (CMS, NMDOH, DNV, etc.).

  • Interviews patients and their families, physicians, and nursing staff to obtain information relevant to medical problems and discharge planning needs.

  • Acts as liaison between the hospitalized patient, their family, their physician and the community health agencies, facilities and allied services that will provide continued care after discharge from hospital.

  • Provides brief counseling services to patients and their families for problems related to hospitalization and future care needs.

  • Maintains current information on all frequently used community resources, financial benefits programs and placement facilities, updating departmental files and handouts for patients.

  • Assists patients and their families in formulating and implementing appropriate plans for post-discharge health care.

  • Assists with nursing home placement inquiries and documentation required for placement problems.

  • Develops Policies & Procedures as needed for specific Social Work needs.
