Short Description

Fluency in Spanish preferred.

 

The Health Coach is an integral part of the healthcare team, assisting in patient care management and care transitions, screening and assisting patients with connecting to food, housing, and other community resources, helping patients gain knowledge, skills, tools, and confidence to become active participants in their care and coaching patients to schedule their own appointments and transportation.

 

The health coach bridges the gap between clinician and patient by providing individual support and assistance in navigating the health care and social service systems to provide continuity of care

 

 

Provide ongoing education and information about disease management and health education as directed by clinicians. 

 

Make follow-up and reminder calls to patients.

 

Document all patient interactions in EPIC including outreach encounters and telephone encounters. 

 

Escalate clinical questions to office nurses or Director of Clinical Practice.

 

Works as part of collaborative team with clinicians, office staff, population health team and community partners to ensure continuity of care for patients.

 

Performs all related duties or special projects as assigned/required.


Experience Required

Experience working in a healthcare office setting, or community outreach and education required. 

 

Knowledge of health promotion, nutrition, and counseling preferred (specific disease and lifestyle related topics ideal).

 

Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.

 

Ability to take direction, follows through with tasks, and work independently.

 

Flexibility, optimism, and a cooperative working style.


Education Requirements

High School Diploma or Equivalent; clinical training (i.e. MA, CNA, LPN) a plus.

 

Associate degree preferred. 


Special Requirements

Fluency in Spanish preferred.

 

Competent in basic computer skills; knowledge of using an electronic health records system preferred.
