Short Description

  

Linkage to care point person for newly identified HIV infected persons enrolling in Cooper EIP medical care services.  Re-engage HIV infected persons lost to care.  Provide partner services to new and existing HIV infected patients.  Work collaboratively with the medical care team providing patient counseling for management of medical treatment care plan.

Employ social networking strategies with new and existing HIV-infected patients to bring additional social contacts in for HIV testing.  Provide prevention counseling/strategies such as CLEAR, ARTAS, or Partners in Health for patients and their families..  

This position’s role is key in the implementation of multi-disciplinary care.


Experience Required

Minimum of 1 year experience in providing HIV prevention or care services to people living with HIV.  Experience counseling HIV (+) clients on connecting to available services. Knowledgeable of community resources


Education Requirements

BSN required, current NJ RN License


License/Certification Requirements

Current NJ-RN License, BSN required


Special Requirements

Division of HIV, STD, and TB Services (DHSTS) clinical navigator trainings (will be provided)
