Responsibilities:

  • Deliver high-quality primary care services directly within the senior living facility clinic.
  • Manage new patient admissions, ensuring smooth transitions and continuity of care.
  • Conduct patient visits, rounding, and complete documentation based on an agreed-upon schedule with facility partners and in response to day-to-day patient needs.
  • Collaborate with a multi-disciplinary care team to address and meet the comprehensive healthcare needs of senior living residents.
  • Maintain exceptional patient care standards in a post-acute care setting, focusing on the unique needs of the elderly population.

Qualifications:

  • MD/DO degree from an accredited medical school.
  • Board Certified in Internal or Family Medicine.
  • Current, unrestricted license to practice medicine in the state of employment.
  • Proficiency with Electronic Medical Records (EMR) systems.
  • Compassionate, intelligent, motivated, and a team player with a strong commitment to providing care to seniors.
