Duties Duties and Responsibilities : 1. Organizational/Administrative Role: Interpret and translate SM limitations from non-military providers. Draft language in web-based system and submit recommendations for SMs with health limitations to profiling military provider for consideration and signature. Assists in ongoing patient care coordination until planned outcomes are achieved. Unless otherwise directed by the DSS, support every weekend Medical Readiness Event (MRE) in the state with appropriate staff to schedule meetings with SM in case management, update unit leadership, attend medical synchronization meetings and may provide dental support. Uses automated systems to document and track a variety of data related to case management and care. Integrates personal data files with other available automated systems to provide information required for briefings, presentations, reports. 2. Consultant/Collaborator Role: Provides advice, guidance, and health teaching to SM and Chain of Command, in individual or group settings designed to assist the SM in understanding, accepting and following medical recommendations. The incumbent assists in coordinating comprehensive health care services to support individual Medical Readiness requirements in support of State medical readiness. Presents, with minimal supervision, cases and summary information to senior leaders such as unit commanders and State Surgeon Staff. Incumbent will hold quarterly reviews with members of the health care team (OSS) and chain of command. Provide guidance and interpretation of physical profile limitations to Unit POCs. Works with the Medical Standards Officer to insure the Soldier transitions at an expected rate through the proposed care/treatment plan. Problem solves with the team, on an individual or system basis to correct problems, prevent negative outcomes, inefficiencies. Provides support, clinical expertise, and coaching to personnel within the State Surgeon's Office and Traditional Drilling Medical Providers within the organization. Participates in a continuing process improvement program for ongoing systematic monitoring and evaluation of patient outcomes to include tracking appropriate data. Works with personnel for processing of line of duty investigations IAW AR 600-8-4, medical boards for the disability evaluation system IAW AR 635-40 and DA PAM 635-40 and composing/processing requirements for Soldiers eligible for care under all programs for injured Soldiers which include but are not limited to RCMC, and INCAP IAW all written guidance and policy. 3. Educator Role: Coordinates and implements health prevention and education programs related to fitness for duty, medical readiness, and deployability This will encompass complex 1:1 interaction with SM due to the sensitive issues that require discretion yet have a profound impact on deployability. Facilitates and coordinates communication and education between service member and all relevant members of the health care team and chain of command to optimize medical readiness and improve coordination with unit commanders on case management issues, community resources, or other issues. Educates the SM for care, appropriate use of their medical facilities and services. Provides education and guidance on applicable medical transition programs IAW current regulatory and statutory guidance through interdisciplinary collaboration, determines the prioritization, goals, and care plan. Maintains documentation and database for outcome evaluation as required. Briefs and seeks support from commanders, senior leaders, and key personnel on prevention, medical readiness, and access to care. 4. Professional Demeanor: Assumes responsibility and accountability for case management of all Soldiers in case load. Integrates management and case management processes to provide a framework for safe, effective, ethical decision-making within each of the case management team's roles. Supports the organizational vision, mission, and goals. Displays deep rooted values in accord with those of the Army and encourages the same in peers, co-workers, and Soldiers. The incumbent shall have team meetings as required based on coordination CM efforts with medical readiness NCO's, Health Systems Specialist Office, and the State Surgeon office. with the Deputy State Surgeon (DSS), the State Surgeon (SS), and the State Dental Officer (SDO) (or appointed representatives) to review all cases in progress. The incumbent shall be responsible to know the following ARNG web-based electronic data systems to include but not limited to: Medical Operations Data System (MODS): Medical Non-Deployable (MND), Dental Classification Module (DENCLASS), Health Readiness Record (HRR), Medical Data Protection System (MEDPROS), Soldiers Patient Locator (SPL), and the Warrior Transition Report (WTR). 5. Performs other as duties as assigned. SPECIALIZED EXPERIENCE : Must possess a minimum of One year (12 months) on-the-job experience at the GS-09 level; AND/OR at least Thirty-six (36) months experience of basic principles and practices related to the management of occupational health care delivery systems; have experiences in planning for and the execution of state occupational health and industrial hygiene programs; have at least six months experience in one of the medical service core functions, which are prescribed as: Health facilities, health plan management, medical resources, health care information, management and information technology, medical logistics, and medical readiness; have demonstrated skills in written and oral communication; have at least six months experience, education, and/or training which provided working knowledge of the principles and practices related to the management of occupational health care delivery systems.
