Duties
The Clinical Pharmacy Specialist is responsible for both direct and indirect patient care, drug therapy monitoring, formulary management, staff education, some prescription inputting, patient counseling, cost containment and acts as Pharmacy's liaison with physicians and other health practitioners.
The duties of the Clinical Pharmacy Specialist include, but are not limited to:
Utilizes a patient-centered approach to improve patient health outcomes through optimizing medication use in collaboration with the patient and other healthcare providers.Provides prospective consultation to physicians, nurses and other health care providers on appropriate drug therapy.Performs initial and periodic assessment of patients' medication therapy, monitors as needed, and makes recommendations or changes (within scope of clinical privileges) as necessary.Documents pharmaceutical care plan and interventions performed in electronic health record and/or facility specific programs.Orders, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy.Performs medication reconciliation and evaluates appropriateness of medications during transitions of care (i.e., inpatient admission, changes in levels of care, etc.) as indicated.Monitors and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures. Actively participates in the VAADERS Adverse Drug Reaction (ADR) reporting system.Supports effective drug usage criteria approved by the national VHA Pharmacy Benefits Management (PBM), VISN PBM and local Pharmacy & Therapeutic (P & T) which help to ensure quality patient care in a cost-effective manner. Ensures appropriate utilization of the VHA formulary by health care providers.Collaboration with physicians and other health care professionals in the design and implementation of drug research protocols, collection of data and in the analysis and interpretation of results.In conjunction with other Clinical Pharmacy Specialists, provides in-service education programs directed to the development of the Pharmacy staff and education of other health care disciplines.Knows and follows administrative requirements of the facility and those specific to the Pharmacy Formulary, investigational drugs, and general distributive functions.Program planning and evaluation for short and long-range clinical goals to ensure these goals are consistent with existing resources and necessary function and programs within Pharmacy Service.Evaluates physician's orders as to appropriateness based on patient medication profiles, inputs information into computer system if assigned, dispenses medication in accordance with dosage form requirements.Work Schedule: Monday-Friday, 7:00am-3:30pmRelocation Incentive: Not AuthorizedPermanent Change of Station (Relocation Assistance): Not AuthorizedEDRP Authorized: Former EDRP participants ineligible to apply for incentive. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Pharmacists assigned to this position must demonstrate the following KSAs:
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.Expert understanding of regulatory and quality standards for their program area.Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.References: VA Handbook 5005/55, Part II, Appendix G15, Licensed Pharmacist Qualification Standard, dated June 7, 2012.Preferred experience:Completion of a Pharmacy Practice or Specialty Residency with a substantial component (>50%) of patient care activities in pharmacotherapy.Three (3) years of practice experience with a substantial component (>50%) of patient care activities in pharmacotherapy.
The full performance level of this vacancy is GS-13.Physical Requirements: Work demands standing (4 hours), walking (4 hours), repeated bending (4 hours), twisting, and carrying/lifting moderately heavy items (15-44 lbs).
