The IDDT Case Manager coordinates all services necessary to improve the quality of life for people with co-occurring, severe mental health and substance use disorders by combining substance abuse services with mental health services.

How you will change lives…


Case Management


- Ensure consumer involvement in the Person Centered Planning Process (PCP).


- Facilitate the implementation of Individual Plan of Service.


- Perform comprehensive, ongoing psychosocial assessments.


- Participate in legal proceedings including petitioning and testifying in clinical emergencies.


- Provide community based services.


- Facilitate the coordination of consumer intakes, discharges, and transfers.


- Communicate with all service providers in Treatment Plan.


- Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.


- Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.


- Provide referral to appropriate community resources.

You have…


- Bachelor’s degree in a human services field from an accredited institution required.


- LLBSW, LBSW, LLMSW, LMSW preferred.


- Must have experience working with individuals dealing with substance issues.


- Valid Michigan Driver’s License.
