*Summary of Duties and Responsibilities* *:* Provides direct care to patients in accordance with Medical Director orders and plan of care. As an On-call after-hour and weekend patient/family triage role, the night nurse provides support, information and skilled nursing care as needed.

*Job Details and Essential Functions:*
* Triages after-hour and weekend calls from patients/families; performs or coordinates home visits as requested/needed.
* Actively participates in coordination of all aspects of patient’s care, in accordance with current professional standards and practice
* Participates in admission and ongoing patient assessments
* Responsible for developing, evaluating and adjusting the patient’s Plan of Care (POC) as indicated
* Contributes to patient and family counseling and education
* Collaborates with physicians and staff regarding patients’ changing condition
* Oversees licensed nurses and CHHA’s in the delivery of patient care
* Performs administrative functions relating to clinical records, care plans, QAPI, safety, infection control and mandatory abuse reporting requirements
* Provides support to patients/families regarding patient care services
* Responsible for establishing and maintaining a safe environment for patients/families, and a safe working environment for self and coworkers.
* Participates in Quality Assurance and Performance Improvement (QAPI) program and Hospice sponsored in-service

* Sensitivity to and ability to interact with various racial, religious, and ethnic cultures in the organization service area
* Ability to solicit sensitive information from patients and their families/caregivers
* Ability to remain calm with patients and their families/caregivers under fast paced, emotional conditions
* Explain complex ideas and procedures so patients and their families/caregivers can understand the issues related to Hospice care
* Knowledge of State and federal Medicare Hospice regulations desirable
* Knowledge of terminal care criteria and end of life issues
* Demonstrate communications and interpersonal skill
* Demonstrate adaptability, organization, attention-to-detail, thoroughness, responsibility and leadership qualities
* Uses independent judgment and decision-making to organize work and establish priorities
* Responsible for professional demonstration of the Hospice of the Foothills mission
* Providing accurate and timely records of critical events
* Cooperation with the Governing Body, Executive Director, physicians, IDT members, and HOF CCH coworkers
* Read, write, and communicate on a professional level
* Perform multiple tasks in a timely manner

* Public speaking, developing relationships with identified partners, groups, businesses with whom Hospice of the Foothills may have or need to establish affiliation

*Education & Experience:*

*Required*
* Two years experience as an RN preferable in Hospice, home health, community health, or oncology unit or in a Medical/surgical area
* Demonstrated competence in Hospice care delivery techniques
* Knowledge of State and Medicare Hospice regulations
* Knowledge of terminal care and end-of-life issues
* Participate in appropriate continuing education
* Expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities, and to attend mandatory educational programs

*Desired*
* Two year’s experience as an RN for an inpatient Hospice Unit
* BSN with public health certificate

*License / Certifications:*

*Required*
* Possess and maintain current California RN license
* Possess and maintain current CPR certificate
* Possess and maintain valid California driver’s license, a reliable automobile and evidence of appropriate liability insurance

*Desired*
* Certified Hospice and Palliative Nurse (CHPN)

Job Type: Part-time

Pay: $43.83 - $55.76 per hour

Expected hours: 15 per week
