Minimum Requirements

  • Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent graduate degree in Physical Therapy.
  • Current State License as a Physical Therapist in Arizona or eligible to obtain before the assignment start date.
  • Proof of current CPR certification.
  • Current driver’s license, reliable transportation, and insurance.
  • 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required.
  • Previous home care experience - Preferred.
  • Ability to perform clinical assessments, develop and revise Plans of Care, document progress notes, and follow physician and interdisciplinary team recommendations.
  • Ability to perform physical tasks including lifting, bending, pulling, pushing, prolonged standing and walking.
  • Ability to work a flexible schedule and travel locally to patient homes.
  • Must read, write and speak fluent English.

Job Summary and Responsibilities

A registered professional who provides physical therapy to patients of all age groups in home health settings. Plans, implements and evaluates patient care plans to restore or maintain patient well being. Plans, organizes and conducts physical therapy treatment programs based on medical referrals and evaluations. Collaborates with the interdisciplinary team.


Key Duties

  • Perform clinical assessments to identify patient needs and appropriateness for services.
  • Document, prepare and complete Plans of Care, progress notes, phone orders, and other clinical records on an ongoing basis. Revise Plans of Care based on evaluations and assessment data.
  • Plan and provide treatment for neuromuscular, neurological and orthopedic disorders based on medical referrals and therapy assessments.
  • Instruct patients and families in transfer techniques and safe use of adaptive equipment such as walkers, wheelchairs, canes, braces, and prosthetic devices.
  • Order appropriate rehabilitative equipment and adapt appliances to meet home limitations and patient needs.
  • Provide patient and family education that is appropriate to age, condition, culture, and learning readiness.
  • Supervise and direct other team members when applicable and perform duties consistent with patient needs.
  • Apply infection control procedures and universal precautions to prevent contamination and disease transmission.
  • Accept clinical assignments consistent with education and competence.
